Effective April 1st, 2026, Empower Aesthetics will offer medical coverage through UnitedHealthcare (UHC), using the Choice Plus Network. We will continue to provide four medical plan options for 2026-2027, giving you flexibility to choose the plan that best fits you and your family's needs. All employees enrolling in benefits for 2026-2027 will receive new ID cards.

Medical Plans:

$1,000 PPO - Deductible: $1,000 Single / $2,000 Family (Click here for Pharmacy PDL)

$5,000 PPO - Deductible: $5,000 Single / $10,000 Family (Click here for Pharmacy PDL)

$3,500 HDHP - Deductible: $3,500 Single / $7,000 Family (Click here for Pharmacy PDL)

$6,750 HDHP- Deductible: $6,750 Single / $13,500 Family (Click here for Pharmacy PDL)

To search for providers prior to registering you can visit whyuhc.com/welcometouhc and follow the steps below: 

• Select "Find A Network Doctor or Hospital"

• Select "Continue"

• Select "Choice Plus"

• Confirm if location is correct. If not, Select "Change Location" and enter Enter Zip Code & Select "Update Location"

• Select Your Care Category and continue prompts to find desired provider type
